ST segment elevation (J point) in ≥2 leads of ≥0,1 mV (≥0,2 mV in V1–V3)
new (presumed) left bundle branch block
ST segment depression in V1–V3 (posterior infarction)
CAVE: Diffuse ST segment depression with ST segment elevation in aVR="left main equivalent" (critical ischemia of the left main, severe triple vessel disease, aortic dissection, severe anemia, etc..).
