Algorithm

  1. Echocardiography:
  • if velocity TI < 2.8 m/s consider other cause.
  • if velocity TI > 2.8 m/s but < 3.4 m/s PH probable.
  1. Chest X-ray.
  2. Blood test (check for connective systemic disease).
  3. Thoracic CT scan.
  4. Gazometry.
  5. Lung functions.
  6. Cardiac catheterism:
  • confirmed PH if echocardigography ambiguous
  • quantified PH
  • drug testing for vasoreactivity (+ if diminution of 10 mmHg of PAPm) useful if idiopathic PAH. If not treat the cause.
