Enoxaparin (Clexane)

Dosage

1 mg/kg SC, every 12 hours if NSTEMI or unstable angina. 30 mg of enoxaparin in IV bolus in addition with a dose of 1 mg/kg SC, then 1 mg/kg SC every 12 hours if STEMI.

Contra-indications

• Hypersensitivity to enoxaparin, heparin and low molecular weight heparins.
• Acute endocarditis.
• Recent CVA.

If increased risk of hemorrhage prefer standard iv heparin (short half-life and reversibility with protamine).
